①地形。道と田から。熊本県に室町時代、熊本県天草郡苓北町富岡、佐賀県佐賀市城内が藩庁の佐賀藩士に江戸時代にあった。広島県府中市高木町、佐賀県佐賀市川副町大字南里では農業に従事していたと伝える。推定では江戸時代。

※秋田県秋田市で江戸時代に福田姓から改姓した事例があった。福田フクダ参照。

※任那系。770年（宝亀元年）に道田連の氏姓を賜った事例があった。位置不詳。

②岐阜県高山市滝町の小字の道田から発祥。同地付近に分布あり。
