①長野県上田市御岳堂付近（旧：依田）から発祥。鎌倉時代に記録のある地名。山梨県西八代郡市川三郷町高田、山梨県南巨摩郡身延町一色で伝承あり。長野県上田市二の丸が藩庁の信濃上田藩士、長野県松本市丸の内が藩庁の松本藩士、東京都千代田区千代田が政庁の江戸幕府の幕臣、山梨県山梨市下井尻に江戸時代にあった。同幕臣に伝承あり。

②余田の異形。兵庫県丹波市では余田姓だったと伝える。推定では江戸時代初期以前。兵庫県丹波篠山市北新町が藩庁の篠山藩士に江戸時代にあった。

③シナ（中国）系。地形。依に「田」を追加。福岡県福岡市中央区で1974年8月24日に帰化の記録あり。本姓は依。依イ参照。
